What to Bring for your First Appointment or Admission

First Appointment
  • Completed medical and history form.
  • Any braces or special equipment your child currently uses.
  • All medications your child is currently taking in their original bottles.
  • Health insurance card or Medicaid card if you have one (This information may be used for services obtained outside of the hospital.  It will not be used for services provided within the hospital.)
  • X-rays related to your child’s problem.
  • Please bring your child’s own shorts and shirt for examination (There should be no metal on any clothing as it interferes with x-rays).
  • Documents related to legal custody of your child.
Surgery Patients
  • Bring all medications your child is currently taking in their original bottles. Include prescription medicine, herbs, vitamins, minerals or nutritional supplements. They will be stored in the medication room and given back to you when you leave.
  • If there are custody concerns, please bring a copy of current custody papers.
  • Copies of medical insurance cards or any third party payer information.
  • Your child’s most recent wheelchair, braces, special shoes, splints, prosthetics or other orthopaedic items (even if your child has outgrown them).
  • The address and phone number of your child’s primary care physician or other care provider who needs a treatment report.
  • A 24-hour supply of any special food or medical equipment your child will need. The hospital will supply such needs after that period.
  • A list of any allergies to medicines, food, or other items such as latex.
  • Special toys, games, pillows or blankets from home to comfort your child. Please write your child’s name on each item.
  • Money for your meals. The patient’s meals will be provided.
  • Money for medicine for when your child goes home. The hospital cannot fill any prescriptions.
  • Personal care items.
What Not To Bring
  • Please do not bring valuables such as large amounts of cash, jewelry, personal music/movie players, and video games.  We cannot be responsible for lost or stolen items.
  • Items containing latex (rubber) are prohibited from the hospital because of severe allergic reactions by some of our patients. Those items include latex balloons, rubber balls, and certain toys and games. If you have questions about an item, please ask before arriving at the hospital.
  • Electrical appliances such as hair dryers, hair curlers or electric razors.
