Joint Commission Right of Notification
of Safety or Quality Concerns

The Shriners Hospitals for Children (SHC) � Philadelphia is committed to patient safety as well as patient and family satisfaction with the care we provide. As such, we are accredited by the Joint Commission  and licensed by the Pennsylvania Department of Health (DOH) whose standards and regulations deal with hospital quality, the safety of patient care, and the safety of the environment in which care is provided.

SHC-Philadelphia is committed to meeting standards and regulations in all of these areas.

SHC-Philadelphia has an open door policy. If you have any questions, comments, or concerns about care received at SHC-Philadelphia, we encourage you to first discuss them with the caregiver, department manager, or hospital leadership.

In addition, if you do not feel your safety concerns are addressed adequately by hospital leadership, you can contact the Pennsylvania Patient Safety Authority by calling 1-800-254-5164.

Also, if the issue is not resolved by the hospital, you are also always free to contact the Joint Commission to raise your concerns. SHC - Philadelphia has an unannounced survey, by the Joint Commission, every three years to assess their compliance to the standards.

You may send your complaint by e-mail, phone, fax or mail to the Joint Commission. Summarize the
issue(s) in one or two pages and include the name and address of the hospital:

When submitting a complaint to the Joint Commission about an accredited organization, you may either provide your name and contact information or submit your complaint anonymously. Providing your name and contact information enables the Joint Commission to inform you of any actions taken in response to your complaint, and also to contact you should additional information be needed.

It is the Joint Commission�s policy to treat your name as confidential information and not to disclose it to any other party. However, it may be necessary to share the complaint with SHC-Philadelphia in the course of the complaint investigation.

Note: The Joint Commission policy forbids accredited organizations from taking retaliatory actions against anyone for having reported quality of care concerns to the Joint Commission.
