Care and Outcome Management
A care and outcome management plan (COMPlan) is a multidisciplinary tool for care management that coordinates patient care and monitors outcomes after discharge.
The goal of COMPlans is to establish a family and patient-centered-care
delivery system that ensures high-quality and
effective care and increases efficiency of community resources. This approach aims to maintain individuals at their
maximum level of comfort and independence through the most
appropriate level of service.
Outreach Clinics
The outreach clinic consists of a medical team from Shriners Hospitals for Children®
that travels to treat patients outside the hospital. The clinic is coordinated
in Montreal by our local team in collaboration with the local medical community
and the Shriners fraternity.
The outreach clinic sees more than 500 patients
a year and this number is growing. Patients seen in an outreach clinic are
referred back to the hospital only if medically necessary; otherwise
they are referred to the next outreach clinic or to a local health facility for continued care.

including Sudbury, Ontario, Gander, Newfoundland, Bathurst, New Brunswick and

Transition Program
The health care system offers a transition program in which
an interdisciplinary team helps prepare children and adolescents with
disabilities for adulthood. The team will assist the child or adolescent in the
first steps leading to independence by helping him or her achieve basic care
skills such as feeding, dressing, toileting, etc. It is important that the
child/adolescent participates with friends and family outside school activities
(going to the mall, shopping, sports and hobbies for instance). Since these
activities require more than the basic skills, the staff will help the
child/adolescent learn about nutrition, sex education, leisure activities,
transportation, etc.
Telemedicine
Telemedicine, also known as Telehealth, is a way for patients to visit
their doctors. Using videoconferencing equipment (televisions, cameras and
microphones), doctors are able to examine the patient and speak with the family even when distance
separates the patient and the doctor.
